What do these symptoms indicate?

They don't indicate anything in particular. Tiredness and malaise are common, and usually the result of stress, over-working or anxiety. If you're waking up in the night that would also make you feel tired. The swollen joints could be caused by trauma, arthritis, autoimmune conditions or infections. Nobody can tell you which one of those it is without doing a thorough physical examination, and quite possibly a series of blood tests. Osteoarthritis usually causes painful swelling and stiffness which over time causes the joints to become more and more stiff. Gout causes short bouts of very intense pain and swelling in joints. Given that it's been going on for three months, I doubt it's what is causing your problems although I can't say for sure. If the joints are hot, painful and stiff, then the swelling could indicate damage to the joints. Swelling without pain and stiffness is usually not serious.
