What could cause a person to completely swell up, she has gout and she is 75. her glucose level was high & 12?

she was given 12 units of insulin and that did not do anything except lower her glucose level slightly. She usually takes a size 2X and she is wearing a 3X jogging suit that is so tight that it almost has to be cut off of her due to the swelling. She is in a nursing facility in PA and they sent her to the emergency room. Just wondering if you might have any idea. Thanks in advance. Concerned in CT about PA Auntie
